Mental Health Services For Children

Case Management Services

Case management (CM) services assist children and adolescents under the age of 18 with serious emotional disturbances or those at risk of serious emotional disturbance and their families with accessing medical, psychiatric, social, emotional, vocational and other supports essential to meeting their basic needs in order to improve and/or enhance their functioning in the community. Specific components of case management services include assessing clinical needs, designing individualized service plans to address the identified needs, referring/linking to related specific service providers, monitoring the provision and effectiveness of these services, assisting the family to redesign the service plan as needed, and supporting the child/family throughout this process.

The frequency/intensity level of the Case Management services provided varies according to identified clinical needs of the child, based on the psychiatric symptoms, history of current emotional/behavioral adaptive functioning, and treatment history. Consumers may be assigned to any of the following levels of care:

Level A CM: Consumers assessed to need this level of care are provided with crisis intervention, case management and psychiatric services as needed. Consumers at this level typically present with mild psychiatric symptoms and/or behavioral dysfunction. Consumers receive approximately 1.5 hours of primarily office-based case management services per month, which includes direct and indirect contact.

Level B CM:  Consumers assessed to need this level of care are provided with an average of 3 hours of service per month. Consumers at this level typically present with moderate psychiatric symptoms and/or behavioral dysfunctions. Services may be provided in the home, school or community settings and include the above listed services.

Level C CM:  Consumers assessed to need this level of care are provided with an intensive level of services listed above. Consumers at this level typically present with more severe psychiatric or behavioral symptoms. Consumers receive an average of 4 hours of primarily field-based case management services per month, along with access to the array of other services as needed.

At agency intake / service opening, consumers are assessed and assigned to the appropriate level of care. Consumers are reassessed by designated staff person(s) at regular intervals and on an as needed basis to determine the need and appropriateness for either continuing the current level of care or recommending a higher or a lower level of care, based upon the criteria listed above. Collaboration between the consumer case manager (Primary Service Provider), psychiatrist, guardian, school personnel and all involved ancillary staff is a key component of the CM service.
