Mental Health Services For Adults
Case Management Services
Case Management services are comprehensive services and supports available to adults with serious mental illness who are in need of assistance with improving their quality of life and further enhancing their integration in the community. Case Management services are provided by Primary Service Providers (PSPs), all of whom are Qualified Mental Health Professionals. Case Management services include but are not limited to:
Linkage to housing, financial entitlement programs, social services, transportation, vocational and employment programs, medical and dental care
Collaboration with the consumer’s psychiatrist and monitoring of psychiatric functioning and status
Referral to and monitoring of optional services, including residential, mental health supports, recovery groups, and psychiatric rehabilitation programs
Case Management services operate from a strengths-based and recovery-oriented perspective. The level and intensity of services vary depending upon the consumer’s clinical need, and services are provided on a continuum that allows maximum flexibility for the individual to move to a higher or lower level of care.
Who is eligible for Case Management services?—————————
An intake appointment can be scheduled by contacting the main RBHA number (804-819-4000).
The intake clinician will assess for and determine eligibility for services based upon the following criteria that includes but is not limited to:
Being an adult aged 18 years or older
Having a major mental health disorder and demonstrating recurrent impairment or limitations due to the disorder
Requiring services for an extended duration
Willing to participate in services at RBHA
Residing within the City of Richmond
How long do services last? ——————————————————
The need for ongoing services is evaluated once every three months based on an individual’s need and desire to continue receiving case management. Some indicators for case closure include but are not limited to:
Consumer chooses to discontinue services
Consumer is assessed as requiring a higher or lower level of care
Consumer moves beyond boundaries of Richmond City
How often are services provided? ———————————————
The frequency of appointments with the assigned PSP will be determined by each consumer, his/her PSP, the treating Nurse Practitioner (as applicable), and/or the treating psychiatrist. This will also depend on current needs and treatment goals. Contact between the consumer and PSP occurs at least once monthly and is focused on assessing progress with meeting individual goals.
